Tell us about your community, create your affiliate login, and read and accept the agreement and promotion rules in the form below. Brooke reviews each application before approval. You will receive an email with the outcome.

Please wait for your approval email before using password recovery, and do not apply again. If your earlier application was sent through our previous form, contact affiliate@alvara.life so we can check your existing record.
Before applying, read the Affiliate Agreement, Promotion Rules, and Privacy Policy. The form requires you to accept the September 15, 2026 web edition. You can save or print the terms.
